  1. AUROC = area under receiver operating curve; CI = confidence interval; H-L = Hosmer-Lemshow chi-squared test; OR = odds ratio.
  2. aH-L P values for the three models were determined by Hosmer-Lemshow chi-squared test in which P > 0.05 indicates good model calibration of predicted vs observed candidemia across low- and high-risk deciles. P values for each variable within the models were determined by each logistic regression model in which a significant value indicates increased risk for candidemia.
  3. bAUROC ≥ 0.70 indicates good model discrimination.
