  1. Logistic regression model selection was conducted from the fitted model using backward stepwise selection with elimination stopping rule set to p value < 0.1. Response options ‘Prefer not to answer’ and ‘I don’t know’ from each independent variable were excluded from the dataset resulting in a sample size of 2799.The overall p value was obtained from each logistic regression analysis and may be interpreted at the statistical significance of the model given all covariates in the model are held constant
