  3. Chronic kidney disease was defined by a creatinine clearance < 30 ml/min
  4. Chronic heart failure was defined by a left ventricular ejection fraction < 40%
  5. ±Hypertension and diabetes were defined as the need for a disease specific medication
  6. Cerebro-vascular disease was defined by a history of stroke or transient ischemic attack
  7. £COPD was defined by a documented FEV1/FVC < 0.7 according to the GOLD criteria
