Six-point ordinal scale

Four-point ordinal scale

0 = no muscle contraction

0 = paralysis

1 = flicker or trace of muscle contraction

1 = severe weakness defined as >50% loss of strength

2 = active movement with gravity eliminated

 

3 = reduced power but active movement against gravity

2 = slight weakness <50% loss of strength

4 = reduced power but active movement against gravity and resistance

5 = normal power against full resistance

3 = normal strength
