Table 9 Dosing of antifungals in paediatric patients

Amphotericin B deoxycholate

1 mg/kg daily

Liposomal amphotericin B

3 mg/kg daily

Lipid complex amphotericin B

3 to 5 mg/kg daily (> 1 month old)

Fluconazole

12 mg/kg daily

Caspofungin

50 mg/m2, with a loading dose of 70 mg/m2 (> 1 year old)

Micafungin

2 mg/kg daily in children if <40 kg

Anidulafungin

1.5 mg/kg/day, with a loading dose of 3 mg/kg/day (in children 2 to 17 years old)

Voriconazole

7 mg/kg every 12 hours, up to age 12 years
